CAMPREV LIABILITY WAIVER · MEDICAL AUTHORIZATION · MEDIA RELEASE
PLEASE READ CAREFULLY BEFORE SIGNING. THIS IS A LEGALLY BINDING DOCUMENT AND MUST BE COMPLETED BEFORE PARTICIPATION IS PERMITTED.
ASSUMPTION OF RISK
I am aware that CampREV involves motocross training, riding, instruction, competition, games, travel, and related activities, and that there are inherent dangers and risks of injury that cannot be eliminated regardless of the level of care and supervision provided.
These risks include, but are not limited to: collisions with other riders, motorcycles, vehicles, spectators, or fixed objects; jumps, ruts, and changing track conditions; weather conditions; mechanical failures; rider error; dehydration; heat-related illness; concussion; traumatic brain injury; paralysis; spinal injury; broken bones; burns; property damage; and death.
I also acknowledge that there may be other dangers or risks not presently known or reasonably foreseeable. I voluntarily accept and assume all such risks for myself and/or my child.
RELEASE OF LIABILITY & INDEMNIFICATION
In consideration of participation in CampREV activities, I hereby release, waive, discharge, and covenant not to sue PanicREV Ministries, CampREV, its directors, officers, employees, volunteers, trainers, contractors, sponsors, host facilities, agents, and representatives from any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses arising out of participation, to the fullest extent permitted by law.
I further release, indemnify, and hold harmless CampREV from any and all liability including claims arising from passive or active negligence.
To the extent the participant engages in activities outside the scope of CampREV from which injury or damage occurs, CampREV will not be held responsible.
PARTICIPANT RESPONSIBILITIES
Participant agrees to:
• Follow all staff instructions at all times.
• Wear required safety equipment as directed.
• Ride only in designated areas.
• Enter and exit the track only at approved locations.
• Report all injuries and unsafe conditions to staff immediately.
• Conduct themselves in a safe, respectful manner at all times.
CampREV reserves the right to remove any participant for unsafe or disrespectful conduct without refund.
I acknowledge it is my responsibility — and my child's, if applicable — to ensure the participant is sufficiently fit and trained to safely participate.
MEDICAL AUTHORIZATION
I certify that the participant is physically capable of participating in CampREV activities and that all known medical conditions have been disclosed.
In the event of injury or illness, I authorize CampREV representatives to obtain emergency medical treatment when reasonable efforts to contact me have been unsuccessful.
I accept full financial responsibility for all medical expenses, transportation costs, ambulance services, and related expenses incurred.
MEDIA RELEASE
I grant PanicREV Ministries and CampREV the perpetual, irrevocable right to photograph, video record, audio record, and otherwise capture the participant's name, image, likeness, voice, and appearance during CampREV activities, and to use such materials in any format — including print, digital, social media, advertising, fundraising, and promotional materials — without compensation.
I waive all related claims for invasion of privacy, right of publicity, copyright violation, or false light.
GOVERNING LAW & GENERAL PROVISIONS
This agreement shall be governed by and interpreted in accordance with the laws applicable in the state where the CampREV event takes place, without regard to conflict-of-law principles.
If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect to the fullest extent permitted by law.
This agreement is intended to provide PanicREV Ministries, CampREV, and the other released parties with the broadest release and protection permitted by applicable law.
This document constitutes the entire agreement between the parties concerning the matters addressed herein. The terms of this agreement are contractual and not a mere recital.
ELECTRONIC SIGNATURE ACKNOWLEDGMENT
By electronically signing this waiver, I acknowledge that I have read and fully understand this agreement, that I am voluntarily giving up substantial legal rights including the right to sue, and that my electronic signature is intended to bind myself, the participant/minor child for whom I am authorized to sign, and our heirs, successors, assigns, and representatives.
If the participant is under 18 years of age, I certify that I am the participant's parent or legal guardian and have authority to enter into this agreement on the participant's behalf.
